Before your 1st Appointment
Before your first appointment you will need to call your insurance or
managed care company and get an authorization number (if required). Most insurance companies
will not pay or back date for unauthorized visits.
Please let your managed care company know at which office you will be scheduling your appointment. I have different contracts with different tax ID for each site. Please leave the authorization number on my voice mail.

You will need to arrive a minimum of 20 minutes prior to your appointment time to fill out paperwork. These forms need to be filled out before I can see you. I would prefer this not run into your therapeutic time. Please arrive early.
