Before your first appointment will you need to call your insurance or
managed care company and get an authorization number (if required). Most insurance companies
will not pay or back date for unauthorized visits. Please let your managed care company know at which office you will be scheduling
your appointment. I have different contracts with different tax ID for each
site.
